TRICARE Prime — The Low-Cost Option

TRICARE Prime is the managed care option with assigned primary care managers and the lowest out-of-pocket costs. If you’re active duty, you pay absolutely nothing for TRICARE Prime. That’s right, zero. It’s one of those benefits that’s easy to take for granted until you see what civilians pay for comparable coverage.

2025 TRICARE Prime enrollment fees (for retirees and their families):

  • Individual: $185.88/year ($15.49/month) — less than most streaming subscriptions
  • Family: $371.64/year ($30.97/month)

TRICARE Prime copays:

  • Primary care visit: $0

TFL provides secondary coverage for retirees and families who have Medicare Part A and Part B. There’s no enrollment fee for TFL itself, but you do have to pay Medicare Part B premiums.

2025 costs:

  • TFL enrollment fee: $0
  • Medicare Part B premium: $185.00/month (standard; higher for high-income beneficiaries)
  • TFL deductible: None beyond Medicare deductibles
  • TFL cost-share: $0 for Medicare-covered services

TFL picks up the remaining costs after Medicare, typically resulting in zero out-of-pocket for most covered services. I’ve talked to retirees who say it’s the best benefit they earned from their service, and I understand why. Try finding coverage this good on the civilian market.

Pharmacy Copays — Where Shopping Smart Really Pays Off

TRICARE pharmacy costs depend on where you fill prescriptions and the drug tier:

Military pharmacy (MTF):

  • All medications: $0. Free. Nothing. This is the way to go if you live near a base.

TRICARE Pharmacy Home Delivery (90-day supply):

  • Generic: $12
  • Brand-name formulary: $34
  • Non-formulary: $68

Retail pharmacy (30-day supply):

  • Generic: $14
  • Brand-name formulary: $38
  • Non-formulary: $68

Pro tip: Using military pharmacies or home delivery saves significant money, especially for ongoing medications. A brand-name medication that costs $38/month at a retail pharmacy would cost $0 at an MTF or about $11.33/month through home delivery. Over a year, that’s hundreds of dollars in savings for a single prescription.

Out-of-Pocket Maximums — Your Safety Net

TRICARE limits your annual out-of-pocket expenses through catastrophic caps, which is genuinely one of the best features of military healthcare:

2025 catastrophic caps:

  • Active duty families: $1,000/year
  • Retiree families (Prime): $4,500/year
  • Retiree families (Select): $4,500/year
  • Reserve/Guard families: $1,000/year

Once you hit the catastrophic cap, TRICARE covers all additional covered expenses for the remainder of the fiscal year (October 1 through September 30). In the civilian world, out-of-pocket maximums of $8,000-$16,000 are common. TRICARE’s caps are dramatically lower.

  • Stay in network: Network providers accept TRICARE rates, which means no surprise balance billing
  • Use preventive care: Annual physicals and screenings are fully covered — take advantage of them
  • Consider Prime first: If you expect significant healthcare needs, Prime’s lower cost-shares usually come out ahead
  • Track your deductibles: Once they’re met, your cost-shares decrease for the rest of the year
